Location: Santa Maria, California
Start Date: 06-03-2026
End Date: 09-02-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across your assigned region
- Complete OASIS assessments to guide individualized care plans
- Collaborate with PT, OT, and Speech colleagues to provide comprehensive rehab services
- Help patients recover from strokes, joint replacements, spinal injuries, and orthopedic conditions
- Document care using Cerner EMR and maintain compliance with home health regulations
- Support patients and families with education and encouragement throughout their recovery journey
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- OASIS documentation experience required
- At least 1 year of experience as a Physical Therapist
- Home health experience preferred but not required
- Comfortable working independently in home-based settings
Knowledge & Skills
- Strong assessment and clinical reasoning skills
- Excellent communication and patient education abilities
- Proficiency with Cerner EMR documentation
- Ability to manage a variety of diagnoses including ortho, stroke, and spinal conditions
- Flexible and adaptable approach to care in home settings
