Roles & Responsibilities
- Deliver personalized physical therapy care across four rural counties
- Conduct comprehensive Oasis Assessments and Start of Care evaluations
- Help patients recover from falls, fractures, and age-related mobility challenges
- Build meaningful connections with elderly patients in their home environments
- Track visits and time using modern tablet technology
Qualifications
- Active Physical Therapist license in Indiana
- Experience with Oasis Assessments and Start of Care documentation
- Comfortable working independently in rural settings
- Strong assessment and clinical reasoning skills
