Physical Therapist - Home Health
Location: Valley City, North Dakota
Start Date: 06-29-2026
End Date: 09-28-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy care to patients in their homes
- Help patients recover from surgeries, falls, and other conditions with personalized treatment plans
- Collaborate with home health and hospice teams to deliver exceptional care
- Travel within a 60-mile radius to serve patients across the community
- Document care using HomeCare/HomeBase EMR system
- Empower patients to regain independence and improve their quality of life
Qualifications
- Current North Dakota Physical Therapy License
- BLS certification required
- 1+ year of previous home health experience
- Valid driver's license and auto insurance
- Strong PT-related clinical skills

- HomeCare/HomeBase EMR proficiency

- Ability to work independently in home settings
