Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services in patients' homes
Conduct start of care assessments and evaluations
Develop and implement personalized treatment plans
Perform recertifications and routine visits for ongoing care
Support patients recovering from CHF, orthopedic conditions, and pneumonia
Collaborate with clinical managers and educators for optimal patient outcomes

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but not required
Valid driver's license and reliable transportation
Strong communication and documentation skills

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Electronic documentation and tablet proficiency
Compassionate bedside manner
Time management and independent work skills
