Roles & Responsibilities
- Provide compassionate physical therapy services directly in patients' homes across Yolo County
- Assess and evaluate each patient's therapeutic needs, mobility, and home environment
- Develop personalized treatment plans that help patients regain strength and independence
- Educate patients and families on therapeutic exercises and safe use of equipment
- Collaborate with physicians and care teams to track progress and adjust care plans
- Supervise home health aides and coordinate with other healthcare professionals
- Document clinical findings and celebrate patient milestones along their recovery journey
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Valid driver's license and reliable transportation for home visits
- Two supervisory references from your specialty area
Knowledge & Skills
- Physical Therapy assessment and treatment planning
- Home environment evaluation and safety assessment
- Therapeutic exercise instruction and gait training
- Patient and family education techniques
- Documentation and clinical reporting
- Medicare and Medi-Cal compliance knowledge
