Roles & Responsibilities

Provide compassionate physical therapy services directly in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs, mobility, and home environment
Develop personalized treatment plans that help patients regain strength and independence
Educate patients and families on therapeutic exercises and safe use of equipment
Collaborate with physicians and care teams to track progress and adjust care plans
Supervise home health aides and coordinate with other healthcare professionals
Document clinical findings and celebrate patient milestones along their recovery journey

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your specialty area

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home environment evaluation and safety assessment
Therapeutic exercise instruction and gait training
Patient and family education techniques
Documentation and clinical reporting
Medicare and Medi-Cal compliance knowledge
