Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Provide compassionate social work services to patients and families
- Conduct thorough psychosocial assessments and develop care plans
- Coordinate care across multidisciplinary teams for optimal patient outcomes
- Support patients through crisis intervention and resource navigation
- Complete required documentation and maintain compliance standards
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities required
- Strong communication and interpersonal skills
Knowledge & Skills
- Expertise in involuntary psychiatric holds and crisis evaluation
- Knowledge of community resources and patient advocacy
- Understanding of care coordination best practices
- Ability to work collaboratively within interdisciplinary teams
