Physical Therapist - PRN

Location: Inglewood, CA

Shift Type: PRN / Per Diem - flexible shifts (days, evenings, and weekends as needed); exact days and times vary based on facility needs

Candidate Pay Rate: $55 - $63 per hour

Roles & Responsibilities

Perform comprehensive evaluations and develop individualized care plans that promote recovery and independence
Deliver therapeutic exercises and functional mobility training to improve resident function and safety
Educate residents and their caregivers on home programs, safety strategies, and recovery expectations
Collaborate closely with the interdisciplinary team to coordinate care and optimize outcomes
Maintain accurate, timely, and compliant documentation to support quality care and regulatory requirements

Qualifications

Active California Physical Therapy (PT) license required
Experience in a Skilled Nursing Facility (SNF) or geriatric rehabilitation preferred
Strong clinical reasoning and hands-on treatment skills
Excellent communication skills for interacting with residents, families, and care teams
Ability to maintain compliant documentation and work within facility processes

Knowledge & Skills

Skilled in physical therapy evaluation and individualized treatment planning
Proficient in therapeutic exercise prescription and functional mobility training
Strong understanding of geriatric rehabilitation principles and SNF practices
Clear, compassionate patient and caregiver education techniques
Reliable documentation practices and familiarity with compliance expectations
Team-oriented approach and effective interdisciplinary communication
