Roles & Responsibilities

• Establish a collaborative practice program with oncologists to ensure optimal patient care
• Conduct staff development programs and educational services for pharmacy and healthcare professionals
• Serve as a subject matter expert in oncology, providing drug information and reviewing care plans
• Round with Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy
• Provide patient-specific medication education using the teach-back method
• Review oncology care plans within the Electronic Health Record
• Identify and evaluate medication-related problems
• Serve as a preceptor to pharmacy residents and students
• Actively participate in Oncology Service Line meetings to improve cancer care

Qualifications

• Doctorate Degree in Pharmacy
• Post-graduate Year 1 Residency or equivalent experience (at least 3 years of clinical pharmacy practice)
• OR post-graduate year 2 residency in Oncology or equivalent experience (at least 3 years of oncology practice)
• Licensed Pharmacist, Board Certified Oncology Pharmacist
• Three years of hospital experience and completion of clinical training and three years of training in oncology
