Roles & Responsibilities

• Evaluate and treat patients using therapeutic activities to improve daily living and work skills
• Provide skilled care across acute care, nursing home (skilled) and outpatient settings
• Develop and implement individualized treatment plans and ADL training
• Document evaluations, progress notes, daily notes, insurance and discharge documentation accurately
• Collaborate with nurses, physicians and therapy team members to ensure continuity of care
• Participate in patient screening, care team meetings and discharge planning
• Maintain professional growth through continuing education and competency activities

Qualifications

• Master of Occupational Therapy required; Doctorate preferred
• National Board Certification in Occupational Therapy (NBCOT) required
• Current Wisconsin occupational therapy license required
• BLS certification required
• Valid driver’s license and reliable transportation
• Minimum 1+ year of related clinical experience
• Strong attention to detail and excellent documentation skills
• Comfortable working across acute care, skilled nursing and outpatient settings

Education

• Master of Occupational Therapy (required)
• Doctorate in Occupational Therapy (preferred)

Knowledge & Skills

• Patient assessment and skilled evaluation techniques
• Treatment planning and ADL training
• Skilled documentation including insurance and discharge notes
• Interdisciplinary collaboration and effective communication
