Roles & Responsibilities

• Deliver nursing care to home health patients across homes, assisted living, senior apartments and board and care facilities
• Perform wound care and manage Negative Pressure Wound Vacuum (NPWT) treatments according to plan of care
• Conduct PT INR testing in the home setting and follow physician orders for anticoagulation monitoring
• Complete thorough documentation using OASIS and HomeCare Homebase (HCHB) EMR to ensure continuity and compliance
• Manage a patient caseload (up to 6 patients/day) efficiently while maintaining high-quality, compassionate care
• Collaborate with unit teams and float as needed to support patient coverage and case continuity
• Communicate clearly with patients, families and interdisciplinary teams; escalate changes in condition promptly
• Adhere to professional dress and safety standards when entering varied home environments

Qualifications

• Valid California LVN/LPN license required
• Current BLS (AHA) certification required
• Active NPI number required
• Minimum 2 years experience in a healthcare setting (community/home health experience preferred)
• Current driver’s license and proof of automobile insurance required
• Two supervisory references at submittal (one within 12 months and one within 3 years worked)
• Candidates must not have been employed (FT, PT, or PRN) by any CommonSpirit/CHI/Dignity facility within the past year; PRN employment through outside firms is acceptable
• Comfortable working in challenging home environments (cramped, hot/cold, or cluttered conditions)

Education

• LVN/LPN diploma or vocational nursing program completion with valid California LVN/LPN license (BSN not required)

Knowledge & Skills

• Home Health clinical experience and solid knowledge of OASIS documentation
• Experience with HomeCare Homebase (HCHB) EMR preferred
• Wound care expertise including Negative Pressure Wound Vacuum (NPWT) familiarity
• Ability to perform PT INR testing and follow anticoagulation protocols
• Strong oral and written communication and organizational skills
• Ability to manage time effectively to complete visits within scheduled shift
• Professional presence and ability to follow dress code and safety procedures
