Roles & Responsibilities

• Provide compassionate, direct patient care consistent with LPN scope of practice
• Administer medications and treatments safely and document accurately
• Monitor and record patient vital signs and report changes promptly
• Collaborate with RNs, physicians, and the interdisciplinary team to support care plans
• Maintain accurate, timely documentation in the electronic health record
• Support patient and family education with clear, empathetic communication
• Adhere to infection control, safety protocols, and organizational policies
• Assist with unit workflow and contribute to a positive patient experience

Qualifications

• Active LPN license in Pennsylvania (required)
• Current BLS/CPR certification (required or obtained upon hire)
• Comfortable with medication administration, IV site monitoring, and basic wound care
• Excellent communication skills and ability to work collaboratively in a fast-paced setting
• Previous LPN experience in acute care or long-term care preferred
• Reliable, professional demeanor and ability to adapt to varying shift needs

Education

• Completion of an accredited Practical Nursing (LPN/LVN) program
• High school diploma or GED

Knowledge & Skills

• Strong fundamentals in patient assessment and clinical documentation
• Medication administration and safety practices
• Familiarity with electronic health record systems and point-of-care charting
• Basic wound care and infection prevention techniques
• Ability to prioritize tasks, multitask, and remain calm under pressure
• Team-oriented mindset and clear patient-centered communication
