Roles & Responsibilities

Provide compassionate direct patient care and assist with daily living activities
Administer medications and treatments safely and according to orders
Perform routine patient assessments and promptly report changes
Document care accurately in the patient record and maintain clear communication
Collaborate with interdisciplinary team members to deliver coordinated care
Educate patients and families about care plans and follow-up needs
Follow facility policies and promote a safe, respectful environment

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (required or obtain upon hire)
Completion of an accredited practical nursing program (diploma or certificate)
Clinical experience in long-term care, med-surg, or similar settings is preferred
Strong communication, critical-thinking, and time-management skills
Ability to pass background checks and meet facility onboarding requirements

Education

Graduation from an accredited Practical Nursing program (diploma or certificate)
Ongoing CEU participation encouraged to maintain licensure

Knowledge & Skills

Medication administration and basic wound care
Patient assessment and vital signs monitoring
Electronic medical record (EMR) documentation skills
Infection control and safety procedures
Teamwork, patient advocacy, and effective communication
