Roles & Responsibilities

Provide compassionate, high-quality nursing care to residents and patients
Administer medications and treatments accurately per care plans
Assess and monitor patient status and promptly report changes
Document care, observations, and interventions clearly in the medical record
Collaborate with nurses, therapists, and support staff to deliver coordinated care
Educate patients and families about care plans, medications, and follow-up
Support infection control, safety protocols, and facility policies

Qualifications

Current LPN license in Pennsylvania (required)
BLS/CPR certification (required or obtained upon hire)
Comfortable administering medications and following established care plans
Strong communication and teamwork mindset
Prior experience in a long term care, skilled nursing, or similar setting is a plus

Education

Licensed Practical Nurse diploma or certificate
High school diploma or GED

Knowledge & Skills

Clinical assessment and basic nursing procedures
Medication administration and documentation
Familiarity with electronic medical records or charting systems
Infection control practices and patient safety
Time management and prioritization in a busy care environment
Empathy, patience, and strong interpersonal skills
