Contract Type: Per-Diem

Shift Type: 8.5-hour shift; specific start/end times and scheduled days to be determined by the facility (schedule provided upon hire)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Deliver compassionate, hands-on nursing care to residents with professionalism and warmth
Administer medications and treatments per physician orders and document accurately
Monitor, assess and record vital signs and resident conditions, escalating concerns appropriately
Maintain accurate, timely documentation in the patient chart/EMR
Collaborate with nurses, nurse aides and interdisciplinary team members to ensure excellent resident outcomes
Support safe infection control and safety protocols to protect residents and staff

Qualifications

Current LPN/LVN license or eligibility to obtain licensure in Pennsylvania
Graduate of an accredited practical nursing program or equivalent clinical training
Active CPR/BLS certification preferred or willingness to obtain upon hire
Strong communication skills and a caring, patient-focused attitude
Reliable, detail-oriented, and able to work effectively within a team

Education

Graduate of an accredited Practical Nursing (LPN/LVN) program
High school diploma or GED

Knowledge & Skills

Medication administration and basic clinical procedures
Resident assessment and care planning
Accurate documentation and charting in clinical records/EMR
Basic wound care and infection control practices
Good time management and ability to prioritize tasks during shift
Collaborative teamwork and clear communication with clinical staff
