Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents and patients
Perform timely medication administration and follow medication safety protocols
Assess patient status, monitor vital signs, and report changes to the care team
Document care accurately and thoroughly in the patient chart/EHR
Assist with wound care, basic treatments, and implementation of care plans
Collaborate with nurses, CNAs, and interdisciplinary team members to ensure excellent outcomes

Qualifications

Active LPN license (Pennsylvania) — required
Completion of an accredited LPN program
Current CPR/BLS certification (or willingness to obtain)
Strong communication skills and a team-first attitude
Previous experience in long-term care or similar setting preferred

Education

Graduate of an accredited LPN/Vocational Nursing program
High school diploma or GED

Knowledge & Skills

Medication administration and medication safety
Basic wound care and dressing changes
Patient assessment and recognition of clinical changes
Accurate charting and familiarity with electronic health records
Infection control, safe patient handling, and documentation best practices
Professional bedside manner and effective communication with patients and families
