Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents as an LPN
- Administer medications and document administration accurately
- Perform routine assessments and report changes in condition promptly
- Collaborate with RNs and interdisciplinary team members to implement care plans
- Maintain accurate and timely patient records and charting
- Support activities of daily living and promote resident dignity and independence
- Participate in infection control practices and maintain a safe care environment
Qualifications
- Current LPN license (Pennsylvania preferred) to practice safely and legally
- Basic Life Support (BLS) certification preferred or willing to obtain
- Completion of an accredited practical nursing program
- Excellent communication skills and a team-oriented attitude
- Previous long-term care or skilled nursing experience is a plus
Education
- Graduate of an accredited Practical Nursing (LPN) program; high school diploma or equivalent required
Knowledge & Skills
- Medication administration and documentation best practices
- Fundamentals of wound care and basic nursing assessments
- Familiarity with electronic health records and charting
- Strong interpersonal skills and the ability to build rapport with residents
- Time management, prioritization, and reliable clinical judgment
