Contract Type: Per-Diem

Shift Type: 12-hour shift (specific start/end times not provided). Schedule to be confirmed with the facility; may include day or night rotations depending on operational needs.

Hours per Week: 36

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and assist with activities of daily living
Administer medications and treatments accurately and document per facility protocols
Perform focused patient assessments and report changes promptly to the RN or provider
Maintain clear, timely, and accurate documentation in the medical record
Collaborate with the interdisciplinary team to support individualized care plans
Support infection control practices and maintain a safe care environment
Communicate effectively with patients and families to provide education and emotional support

Qualifications

Current, active LPN license in Pennsylvania (required)
CPR/BLS certification or ability to obtain upon hire
Current immunizations per facility policy (e.g., flu, COVID-19 as applicable)
Strong clinical judgment, compassion, and patient-focused attitude
Previous experience in long-term care or similar settings is preferred but not required

Education

Vocational nursing diploma or equivalent practical nursing education

Knowledge & Skills

Skilled in medication administration and basic nursing procedures
Comfortable with patient assessment, vital signs, and documentation
Familiarity with electronic medical records and charting best practices
Excellent communication and teamwork skills
Ability to prioritize tasks and manage time effectively during a busy shift
