Roles & Responsibilities

Assess resident conditions and report changes promptly to the care team
Administer medications and treatments accurately and safely
Monitor vital signs and document observations thoroughly
Collaborate with nurses and interdisciplinary staff to deliver excellent care
Provide compassionate, resident-centered assistance with daily needs
Educate residents and families about care plans and procedures
Maintain accurate clinical documentation in the medical record
Support a safe, clean environment and follow infection control practices

Qualifications

Licensed Practical Nurse (LPN) license or eligible for licensure in Pennsylvania
Current Basic Life Support (BLS/CPR) certification preferred
Previous LPN experience in long-term care or similar settings preferred
Strong communication skills and ability to work as part of a team
Reliable, professional, and committed to resident-centered care

Education

Completion of an accredited Practical Nursing program or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Basic clinical assessment and vital sign monitoring
Familiarity with electronic medical records and charting
Infection control, safety procedures, and resident rights
Effective interpersonal skills and teamwork
