Roles & Responsibilities

• Deliver compassionate, resident-centered nursing care that preserves dignity and comfort
• Administer medications and treatments accurately and document per policy
• Monitor and report changes in patient condition to the care team promptly
• Support care planning and participate in effective handoffs for continuity
• Perform wound care, vital signs, and routine assessments with attention to detail
• Maintain accurate electronic medical records and ensure compliance with documentation standards
• Promote infection control and safety practices to protect residents and staff
• Collaborate with nurses, therapists, and support staff to achieve the best outcomes

Qualifications

• Valid LPN license in Pennsylvania (required)
• Graduate of an accredited practical/vocational nursing program
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Ability to pass background check and drug screening as required
• Strong communication skills and a team-oriented attitude
• Reliable, punctual, and committed to delivering high-quality care

Education

• Graduate of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital sign monitoring
• Wound care, ADL assistance, and resident hygiene support
• Familiarity with electronic medical records and charting
• Infection control principles and safe work practices
• Time management, prioritization, and teamwork in a clinical setting
