Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and safely
Monitor patient vital signs and report changes promptly to the care team
Collaborate with RNs and interdisciplinary staff to implement care plans
Document patient care, observations, and interventions in charts/EHR
Support resident comfort, dignity, and activities of daily living
Participate in safety, infection control, and quality improvement efforts

Qualifications

Current LPN license in Pennsylvania (or compact/state-appropriate) — required
CPR/BLS certification — current and active
Prior LPN experience in long-term care, skilled nursing, or similar clinical setting preferred
Strong communication and teamwork skills; patient-centered mindset
Ability to pass background checks and meet facility onboarding requirements

Education

Graduate of an accredited Practical Nursing (LPN) program
High school diploma or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Basic and focused patient assessment skills
Familiarity with infection control and safety protocols
Comfortable charting and maintaining accurate patient records
Ability to work efficiently in a fast-paced clinical environment
