Roles & Responsibilities

Provide compassionate, high-quality nursing care to assigned residents
Administer medications and treatments accurately and document appropriately
Monitor resident status and report changes to the interdisciplinary team promptly
Support care planning and coordinate with CNAs, RNs and other staff for seamless care
Educate residents and families with clear, respectful communication
Maintain accurate clinical records and ensure compliance with facility policies

Qualifications

Valid LPN license in New York required
Current BLS/CPR certification required or must obtain upon hire
Previous long-term care or skilled nursing experience preferred
Strong communication and teamwork skills with a resident-centered mindset
Ability to pass background checks and meet facility health requirements

Education

Practical Nursing diploma or certificate from an accredited program

Knowledge & Skills

Medication administration and documentation best practices
Basic assessment and vital signs monitoring
Wound care basics and assistance with ADLs
Familiarity with long-term care regulations and charting
Comfortable working in a fast-paced team environment
