Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and document accordingly
Monitor patient status and promptly report changes to the care team
Collaborate with nurses, therapists, and support staff to coordinate care plans
Assist with admissions, discharges, and transfers in a professional manner
Maintain clean, safe, and organized patient areas while following facility protocols

Qualifications

Current LPN license in good standing (license requirements follow state rules)
Completion of an accredited practical nursing program
Basic Life Support (BLS) / CPR certification (or willingness to obtain)
Strong communication skills and a patient-centered mindset
Previous experience in long-term care or acute care is a plus

Education

Completion of an accredited LPN/LVN program; high school diploma or GED required

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital sign monitoring
Infection control and universal precautions
Electronic charting and basic computer skills
Teamwork, time management, and prioritization in a fast-paced setting
