Roles & Responsibilities

Provide compassionate, hands-on nursing care as an LPN to residents and patients
Administer medications and treatments accurately and on schedule
Monitor patient status, document changes, and escalate concerns to the care team
Collaborate with RNs, physicians, and support staff to deliver coordinated care
Assist with admissions, transfers, and discharge planning when needed
Maintain accurate, timely charting and uphold facility policies and regulatory requirements

Qualifications

Current, active LPN license in good standing (GA license preferred)
Basic Life Support (BLS) / CPR certification or willingness to obtain upon hire
Demonstrated clinical experience as an LPN; prior long-term care or hospital experience a plus
Ability to pass background check and drug screen as required
Strong communication skills and a patient-centered mindset

Education

LPN diploma or equivalent from an accredited nursing program

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care, vital signs monitoring, and patient assessment
Familiarity with electronic charting and clinical documentation
Ability to prioritize tasks and work effectively in a fast-paced environment
Team collaboration and clear communication with interdisciplinary staff
