Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with care plans
• Administer medications and treatments accurately and safely
• Monitor and document resident condition and communicate changes to the care team
• Support activities of daily living and promote resident comfort and dignity
• Collaborate with interdisciplinary teams to deliver high-quality care
• Assist with admissions, discharges, and care transitions as needed
• Participate in safety, infection control, and quality improvement initiatives

Qualifications

• Active Licensed Practical Nurse (LPN) license (required)
• Current BLS/CPR certification preferred or willingness to obtain promptly
• Comfortable administering medications and following established nursing protocols
• Experience in long-term care, skilled nursing, or a related healthcare setting is a plus
• Strong communication skills and a patient-centered approach
• Reliable, professional, and able to pass facility onboarding requirements

Education

• Completion of an accredited practical nursing program (LPN diploma or certificate)
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital signs monitoring
• Familiarity with care planning and nursing documentation systems
• Infection control standards and safe patient handling
• Team collaboration and clear interpersonal communication
• Time management and prioritization in a fast-paced care environment
