Roles & Responsibilities
- Provide compassionate bedside care to residents in a skilled nursing setting
- Administer medications and treatments accurately and safely
- Monitor, record, and report changes in patient condition and vital signs
- Collaborate with nurses, therapists, and physicians to deliver coordinated care
- Maintain accurate and timely documentation in the patient record
- Assist with admissions, discharges, and patient transfers smoothly and professionally
- Participate in quality improvement, infection control, and safety initiatives
Qualifications
- Active LPN license in Pennsylvania (required)
- Current BLS/CPR certification (required or obtain upon hire)
- Completion of an accredited Practical Nursing program
- Ability to pass background check and drug screen
- Strong communication skills and a team-oriented mindset
- Previous experience in long-term care or skilled nursing preferred
Education
- Graduation from an accredited Practical Nursing program (or equivalent)
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment and monitoring of vital signs
- Wound care and infection control principles
- Proficiency with electronic medical records or willingness to learn
- Time management, prioritization, and the ability to multitask
- Compassionate patient- and family-centered communication
