Roles & Responsibilities

Provide compassionate direct nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and safely
Monitor resident condition and report changes to the care team promptly
Document assessments, interventions, and outcomes in the patient record
Collaborate with RNs, therapists, and support staff to create individualized care plans
Assist with activities of daily living while promoting resident dignity and independence
Participate in unit meetings and contribute to quality improvement efforts

Qualifications

Current LPN/LVN license in Pennsylvania
Successful completion of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification or willing to obtain
Current immunizations and ability to pass background checks and drug screening
Excellent communication skills and a resident-centered approach
Previous experience in skilled nursing or long-term care preferred

Education

Completion of an accredited practical nursing program
High school diploma or equivalent

Knowledge & Skills

Knowledge of basic nursing principles and clinical assessment
Medication administration and documentation best practices
Familiarity with electronic health records and charting
Ability to prioritize tasks in a fast-paced care environment
Strong interpersonal skills for working with residents and families
Basic wound care and infection control practices
