Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and daily nursing services
Administer medications and treatments according to plan of care
Monitor and document patient conditions and report changes promptly
Collaborate with nursing team and interdisciplinary staff to deliver quality care
Support residents with activities of daily living while promoting dignity
Participate in care planning and contribute clinical insights

Qualifications

Valid Licensed Practical Nurse (LPN) license in Pennsylvania (required)
Current Basic Life Support (BLS) / CPR certification (or willing to obtain)
Comfortable providing hands-on care in a skilled nursing environment
Clear background check and up-to-date immunizations as required by facility
Strong communication skills and a team-first attitude
Previous LPN experience in long-term care or skilled nursing preferred

Education

Practical Nursing diploma or certificate and active LPN license

Knowledge & Skills

Knowledge of medication administration and basic nursing procedures
Familiarity with resident care documentation and charting
Ability to assess patient needs and escalate concerns appropriately
Good time management and organizational skills during shift work
Compassionate bedside manner and strong interpersonal skills
