Roles & Responsibilities
- Provide compassionate direct patient care and assist with daily living activities
- Administer medications and treatments accurately and document care thoroughly
- Monitor patient status and report changes to the care team promptly
- Collaborate with nurses, therapists, and support staff to create a person-centered plan
- Support admissions, discharges, and transfers with professionalism and attention to detail
- Maintain a safe, clean environment and follow infection control and facility policies
Qualifications
- Current LPN license (Licensed Practical Nurse) — Pennsylvania licensure preferred
- Graduate of an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification or willingness to obtain
- Ability to pass background check and drug screening as required by the facility
- Strong communication skills and a compassionate approach to patient care
Education
- Graduate of an accredited Practical Nursing program (LPN)
- High school diploma or GED required
Knowledge & Skills
- Medication administration and accurate charting
- Vital signs assessment and basic clinical monitoring
- Familiarity with long-term care / skilled nursing workflows
- Effective time management and ability to prioritize patient needs
- Teamwork, empathy, and clear communication with patients and families
