Roles & Responsibilities
- Deliver compassionate, patient-centered nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document thoroughly
- Monitor resident condition and communicate changes to the interdisciplinary team
- Assist with activities of daily living while promoting resident dignity and independence
- Participate in care planning and contribute clinical observations to support positive outcomes
- Provide family-centered communication and education with empathy and clarity
Qualifications
- Current, active LPN license in New York (required)
- Valid CPR/BLS certification (preferred)
- Comfortable working in a long-term care/skilled nursing environment
- Strong communication skills and the ability to work well on a multidisciplinary team
- Professional, dependable, and patient-focused approach to care
Education
- Vocational Nursing Diploma or Practical Nursing Certificate (required)
Knowledge & Skills
- Skilled in medication administration and documentation
- Basic assessment skills and clinical observation
- Familiarity with long-term care regulations and best practices
- Good infection control and safety practices
- Strong time management and prioritization in a busy clinical setting
