Roles & Responsibilities

• Provide direct and indirect skilled nursing care to long-term and post-acute residents
• Perform IV therapy, wound care and geriatric assessment with clinical accuracy
• Tailor care plans for residents with Alzheimer's and dementia using verbal and non-verbal communication techniques
• Collaborate with RNs, therapists and interdisciplinary team members to deliver safe, comfortable, person-centered care
• Float between units as needed to maintain continuity of care across the facility
• Document clinical observations, interventions and resident progress clearly and promptly
• Communicate effectively with residents, families and team members to support therapeutic outcomes

Qualifications

• Current Maine Licensed Practical Nurse (LPN) license or compact LPN license required
• Associate degree in Nursing or graduation from a Maine State Board-approved LPN program
• 1-3 years of LPN/RN experience preferred, ideally in long-term care or rehab settings
• Competence with IV therapy, wound care and geriatric assessment preferred
• Strong teamwork and communication skills; ability to work well within an interdisciplinary team
• Flexibility to provide 24/7 coverage and to float to other units as needed
• Medical terminology knowledge preferred

Education

• Associate in Nursing OR Graduate of a Maine State Board-approved LPN course

• Clear clinical documentation and care planning
