Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope
• Administer medications and treatments safely and document according to policy
• Perform thorough patient assessments and report changes to the care team
• Maintain accurate and timely clinical documentation in the medical record
• Collaborate with nurses, providers, and support staff to deliver coordinated care
• Support patient and family education with clear, respectful communication
• Participate in orientation and ongoing training to maintain clinical competencies

Qualifications

• Active LPN license in New Hampshire (required)
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) certification or willingness to obtain
• Demonstrated ability to work 12.5-hour clinical shifts and adapt to per diem scheduling
• Strong communication skills and ability to work effectively on a collaborative care team
• Current immunizations and compliance with facility credentialing requirements

Education

• Graduate of an accredited practical nursing (LPN) program
• Active LPN licensure in New Hampshire

Knowledge & Skills

• Medication administration and monitoring
• Patient assessment and vital signs interpretation
• Wound care fundamentals and basic clinical procedures
• Proficiency with electronic medical records and documentation
• Time management, prioritization, and clinical judgment
• Professional communication and teamwork in a healthcare setting
