Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents and patients
- Administer medications and treatments accurately and document appropriately
- Perform routine assessments, monitor vitals, and report changes in condition
- Collaborate with RNs, physicians, and interdisciplinary team members to support care plans
- Maintain clear, timely documentation in the patient record
- Follow infection control and safety protocols to protect patients and staff
- Support patient and family education with empathy and clarity
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
- Current BLS/CPR certification (or willing to obtain)
- Completion of an accredited Practical Nursing program (diploma or certificate)
- Strong clinical judgment, communication skills, and patient advocacy
- Ability to pass background check and meet facility onboarding requirements
Education
- Practical Nursing diploma or certificate (LPN program)
- High school diploma or GED
Knowledge & Skills
- Medication administration and accurate charting
- Patient assessment and monitoring of vital signs
- Documentation and familiarity with electronic health records
- Infection control practices and safety procedures
- Teamwork, time management, and effective communication with patients and families
