Roles & Responsibilities

• Provide compassionate bedside care to residents, helping with daily living activities and promoting dignity
• Administer medications and treatments accurately, following facility policies and care plans
• Monitor and document vital signs, changes in condition, and resident responses to interventions
• Communicate clearly with the interdisciplinary care team and families to support coordinated care
• Assist in developing and implementing individualized care plans to meet resident goals
• Maintain a safe, clean environment and adhere to infection control and safety protocols
• Support admissions, discharges, and transfers with organized, resident-focused handoffs

Qualifications

• Current, active LPN license in Pennsylvania (required)
• Completion of an accredited Practical Nursing program
• Basic Life Support (BLS) certification from an approved provider (preferred or obtain upon hire)
• Comfortable with medication administration, documentation, and basic clinical assessments
• Strong communication and teamwork skills, with a compassionate approach to resident care
• Previous long-term care or skilled nursing experience is a plus

Education

• Completion of an accredited Practical Nursing (LPN) program
• High school diploma or GED

Knowledge & Skills

• Knowledge of practical nursing fundamentals and long-term care best practices
• Skilled in medication administration and accurate clinical documentation
• Ability to perform vital sign monitoring and basic clinical assessments
• Familiarity with infection control standards and resident safety protocols
• Good time management and prioritization in a busy clinical environment
• Strong interpersonal skills for working with residents, families, and care teams
