Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents in a long-term care setting
• Administer medications and treatments according to physician orders and facility policies
• Perform regular patient assessments and accurately document changes in condition
• Collaborate with the nursing team, nursing leadership, and interdisciplinary staff to support resident care plans
• Assist with activities of daily living while promoting resident dignity and independence
• Communicate clearly with residents, families, and providers to ensure coordinated care
• Maintain accurate, timely charting and adhere to infection control and safety protocols

Qualifications

• Active LPN license in Pennsylvania (required)
• Completion of an accredited Practical Nursing program (LPN diploma or certificate)
• Current CPR/BLS certification (or willingness to obtain)
• Comfortable working 8.5-hour shifts in a long-term care/skilled nursing environment
• Preferred: prior experience in long-term care or skilled nursing settings
• Ability to pass required background checks and health screenings

Education

• Practical Nursing diploma or certificate (completion of LPN program) and active LPN license

Knowledge & Skills

• Competent in medication administration and resident monitoring
• Strong clinical assessment and documentation skills
• Knowledge of infection control standards and patient safety practices
• Familiarity with electronic health records and point-of-care charting
• Excellent communication and teamwork skills with residents and families
• Ability to prioritize tasks in a fast-paced care environment
