Roles & Responsibilities

Provide compassionate direct patient care, supporting daily living activities and promoting dignity
Administer medications and treatments accurately, following facility policies and care plans
Collaborate with RNs and interdisciplinary team members to ensure seamless continuity of care
Monitor, document, and report changes in patient condition promptly and professionally
Assist with admission, transfer, and discharge processes to keep care transitions smooth
Support residents' emotional and social needs, building trusting relationships with patients and families
Follow infection control and safety procedures to protect residents and staff

Qualifications

Current, active LPN license in Pennsylvania
Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
Comfortable working in a long term care environment with strong patient-focused bedside skills
Ability to pass required background checks and health screenings
Excellent communication skills and a collaborative mindset

Education

Practical Nursing diploma or equivalent

Knowledge & Skills

Medication administration and charting accuracy
Vital signs monitoring and basic clinical assessment
Knowledge of long term care best practices and elder care needs
Strong documentation and electronic health record familiarity
Patient-centered care, teamwork, and time management
