Roles & Responsibilities

• Provide compassionate, patient-centered nursing care following plan of care and facility policies
• Administer medications and treatments safely and accurately
• Monitor and document patient conditions, vital signs, and responses to interventions
• Communicate effectively with patients, families, and the interdisciplinary care team to coordinate care
• Assist with activities of daily living and promote patient comfort and dignity
• Participate in admission, transfer, and discharge processes with attention to detail
• Report changes in patient status promptly and escalate concerns to RN/MD as appropriate
• Contribute to a safe, clean care environment and adhere to infection control standards

Qualifications

• Current Licensed Practical Nurse (LPN) license — required
• CPR/BLS certification — current
• Previous experience in a clinical setting preferred but new grads with strong skills are welcome
• Ability to work 12+ hour day shifts and adapt to per diem scheduling
• Strong communication, time management, and patient advocacy skills
• Ability to perform physical aspects of nursing care (lifting, transferring, standing for shifts)

Education

• Graduate of an accredited Practical Nursing program
• Active LPN license in good standing

Knowledge & Skills

• Medication administration and basic pharmacology knowledge
• Proficient in patient assessment and vital sign monitoring
• Familiarity with electronic health records and accurate charting
• Wound care, dressing changes, and basic clinical procedures
• Infection control practices and safe patient handling
• Excellent interpersonal skills and a collaborative mindset
