Roles & Responsibilities

Provide compassionate direct patient care and assist with daily living activities
Administer medications and treatments according to care plans and facility policy
Monitor patient status, document observations, and communicate changes promptly
Collaborate with nurses and interdisciplinary team members to deliver excellent care
Support admissions, discharges, and smooth transitions of care with professionalism
Maintain accurate and timely charting to ensure continuity and quality of care

Qualifications

Current, active LPN license (Licensed Practical Nurse) in good standing
Demonstrated ability to work 12-hour day shifts and maintain stamina for extended hours
Basic life support (BLS/CPR) certification preferred or willingness to obtain
Comfortable with medication administration, documentation, and teamwork
Professional, reliable, and patient-focused attitude for long-term patient relationships

Education

Completion of an accredited LPN program
High school diploma or GED

Knowledge & Skills

Strong fundamentals in patient assessment and basic nursing care
Medication administration and safe handling practices
Clear and compassionate communication with patients, families, and staff
Accurate clinical documentation and charting skills
Ability to prioritize tasks in a fast-paced clinical environment
