Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and document appropriately
- Monitor and report changes in patient condition to the care team
- Collaborate with nurses and interdisciplinary staff to deliver person-centered care
- Assist with daily living activities and promote patient dignity and independence
- Maintain clear, timely charting and ensure accurate handoffs during shift changes
Qualifications
- Current Licensed Practical Nurse (LPN) license valid in Connecticut
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) certification or willingness to obtain promptly
- Reliable clinical skills in medication administration, charting, and basic assessments
- Strong communication skills and a team-player attitude
Education
- Completion of an accredited LPN practical nursing program
- High school diploma or GED
Knowledge & Skills
- Medication administration and documentation best practices
- Fundamental nursing assessments and vital signs monitoring
- Infection control and safe patient handling
- Electronic charting or paper charting workflows
- Effective communication with residents, families, and care teams
- Time management and the ability to prioritize in a busy setting
