Roles & Responsibilities

Conduct comprehensive psychosocial assessments across the lifespan to identify needs, strengths, and risk factors.
Develop individualized, evidence-based treatment plans that align with patient goals and integrated care approaches.
Deliver individual and family therapy using evidence-based interventions for depression, anxiety, trauma, substance use, and adjustment disorders.
Collaborate closely with primary care providers, nurses, and behavioral health team members to ensure coordinated, whole-person care.
Coordinate care and referrals to community resources and support services to improve access and follow-through.
Maintain timely, accurate clinical documentation to support continuity of care and regulatory compliance.
Advocate for underserved populations and contribute to efforts that enhance behavioral health services in rural communities.

Qualifications

Master's degree in Social Work from an accredited program (required)
Active LISW license in Iowa (required)
Minimum of 2 years of clinical experience preferred (behavioral health experience highly valued)
Experience working in rural or medically underserved settings is a plus
Strong interpersonal, organizational, and clinical documentation skills

Education

Trauma-informed care and substance use treatment knowledge
