Roles & Responsibilities

Provide comprehensive outpatient primary care for patients across the lifespan with compassion and clinical excellence
Develop long-term, trust-based relationships with patients and their families to improve community health
Diagnose and manage acute and chronic conditions using evidence-based medicine
Coordinate care with experienced clinical and administrative staff to ensure smooth, patient-centered workflows
Participate in quality improvement and community health initiatives that make a real local impact
Document care accurately and timely to support continuity and patient safety

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for medical licensure in New York State
DEA certification or eligibility
A passion for community-focused, patient-centered primary care

Education

MD or DO from an accredited medical school

Knowledge & Skills

Outpatient-focused family medicine
Primary care across the lifespan
Patient-centered medicine and relationship-based care
Collaborative, team-based approach to clinical care
Experience delivering care in community or rural settings
