Roles & Responsibilities

• Provide compassionate, direct patient care and assistance with activities of daily living
• Assist residents with personal hygiene, dressing, toileting, and mobility needs
• Take and accurately record vital signs and report changes in condition to nursing staff
• Support safe patient transfers and positioning while following facility protocols
• Maintain a clean, safe, and dignified environment for residents
• Communicate clearly and kindly with residents, families, and team members to ensure continuity of care
• Document care activities accurately and timely in the resident chart or electronic record

Qualifications

• Active Certified Nursing Assistant (CNA) certification in Pennsylvania required
• High school diploma or GED preferred
• Current CPR/BLS certification preferred or willing to obtain
• Ability to pass background checks and meet facility onboarding requirements
• Compassionate patient focus, strong communication skills, and ability to work collaboratively
• Reliable attendance and ability to perform physical aspects of the role, including lifting and standing

Education

• High school diploma or GED preferred
• Active CNA certification (required)

Knowledge & Skills

• Strong foundational patient care skills and familiarity with ADLs
• Basic understanding of infection prevention and safe patient handling
• Accurate vital signs measurement and documentation
• Comfortable working with interdisciplinary care teams
• Good observational skills with ability to detect changes in patient status
• Customer service mindset when interacting with residents and families
