Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Monitor and record vital signs, intake/output, and patient condition accurately
• Assist with safe transfers, ambulation, and positioning to support mobility
• Communicate changes in patient status promptly to the nursing team
• Maintain a clean, safe, and respectful environment for residents and staff
• Support care plans by documenting observations and participating in team handoffs
• Follow infection control protocols and facility policies to protect patients
• Build trusting relationships with patients and families through respectful communication

Qualifications

• Active CNA certification (or eligible) — state certification required
• CPR/BLS certification preferred or ability to obtain upon hire
• Previous experience in long-term care or clinical setting is a plus
• Ability to pass background check and meet facility immunization requirements
• Strong communication skills and a patient-centered mindset
• Ability to perform physical aspects of the role, including lifting and transfers

Education

• High school diploma or equivalent
• Completion of an accredited CNA training program

Knowledge & Skills

• Basic nursing care and resident support
• Vital sign measurement and patient observation
• Infection control practices and PPE usage
• Documentation skills and familiarity with electronic charting (or willingness to learn)
• Teamwork, time management, and effective communication with patients and staff
