Roles & Responsibilities

• Provide direct, compassionate patient care including medication administration and monitoring
• Perform regular patient assessments and communicate changes to the care team
• Document care and chart accurately in the facility’s patient records
• Collaborate with RNs, physicians, and multidisciplinary staff to coordinate safe, effective care
• Support patient education and discharge planning so patients leave informed and confident
• Maintain infection control, safety protocols, and facility policies to protect patients and staff
• Assist with activities of daily living and provide emotional support to patients and families

Qualifications

• Valid Licensed Practical Nurse (LPN) license in New Mexico (or compact licensure where applicable)
• Current BLS/CPR certification required; ACLS or PALS if applicable/preferred
• Previous experience in acute care, rehab, or long-term care is a plus
• Strong communication skills and ability to work effectively on a care team
• Ability to pass required background checks and meet facility onboarding requirements

Education

• LPN diploma, certificate, or equivalent from an accredited program
• For RN-classified roles: an Associate’s or Bachelor’s degree in Nursing (if applying as RN)

Knowledge & Skills

• Medication administration and basic clinical assessment skills
• Familiarity with electronic medical records and accurate documentation
• Patient safety, infection control, and quality improvement mindset
• Time management, prioritization, and the ability to handle a fast-paced environment
• Compassionate bedside manner and strong interpersonal skills
