Roles & Responsibilities
- Provide compassionate direct patient care, assisting with activities of daily living
- Monitor and report changes in patient condition to the nursing team
- Assist with safe transfers, mobility, and positioning of patients
- Maintain accurate patient documentation and support care plans
- Support infection control and facility safety protocols at all times
- Collaborate with nurses and interdisciplinary team members to deliver quality care
Qualifications
- Current CNA certification in the practicing state (required)
- Active basic life support (BLS/CPR) certification preferred or willing to obtain
- Demonstrated ability to provide patient-centered care with dignity and respect
- Previous experience in long-term care or rehabilitation settings is a plus
- Valid state-required background checks and health screenings as applicable
Education
- CNA certification (required)
- High school diploma or GED (preferred)
Knowledge & Skills
- Strong hands-on patient care skills and safe body mechanics
- Clear, compassionate communication with patients and coworkers
- Basic charting and documentation skills
- Ability to follow care plans, safety procedures, and infection control standards
- Team player mindset with reliability and attention to detail
