Roles & Responsibilities

• Perform comprehensive speech-language evaluations to identify patient needs and set meaningful goals
• Develop and implement individualized treatment plans that drive measurable progress
• Provide daily treatments and hands-on therapy to improve communication, cognition, and swallowing function
• Plan and execute timely discharges with clear recommendations for ongoing care and caregiver education
• Document thoroughly and maintain productivity expectations in line with facility standards
• Collaborate closely with an interdisciplinary team to ensure seamless, patient-centered care
• Follow facility policies and CRG procedures to maintain high standards of safety and compliance

Qualifications

• 1-3 years of clinical experience (as indicated) with demonstrated SLP skills
• Prior Skilled Nursing Facility (SNF) experience strongly preferred
• Active SLP credentials and state licensure as required for practice (please hold or be eligible)
• Comfortable working within productivity and documentation expectations (therapist productivity requirement noted)
• Professional appearance and willingness to follow facility dress code (black scrubs)

Knowledge & Skills

• Skilled in Speech-Language Pathology evaluation and treatment planning
• Experience with discharge planning and transition recommendations
• Familiarity with SNF workflows and interdisciplinary collaboration
• Strong documentation, time management, and patient communication skills
• Ability to work effectively in a team-oriented therapy environment
