Roles & Responsibilities

• Provide skilled speech-language pathology evaluations and individualized treatment for SNF residents
• Develop and implement measurable treatment plans that drive meaningful progress
• Document clinical care accurately and timely, meeting facility productivity and documentation expectations
• Collaborate with nursing, rehab, and interdisciplinary teams to coordinate patient-centered care
• Educate residents and families on communication, cognition, and swallowing strategies
• Adhere to Continuum Rehabilitation Group and facility policies and procedures while maintaining professional standards

Qualifications

• Licensed SLP in New Mexico or ability to obtain state licensure prior to start
• 1-3 years of clinical experience in adult/geriatric settings preferred
• Experience in skilled nursing facility practice and familiarity with SNF documentation requirements
• Comfort meeting productivity expectations (90% productivity requirement)
• Ability to work onsite, wear facility dress code (black scrubs), and be present in building to receive guaranteed hours
• Strong interpersonal skills and a team-first attitude

Knowledge & Skills

• Skilled in adult speech, language, cognitive-communication, and swallowing evaluations and interventions
• Solid clinical reasoning and treatment planning for geriatric populations
• Clear, efficient clinical documentation and time management
• Collaborative communication with interdisciplinary teams and families
• Ability to follow facility policies, productivity tracking, and clinical protocols
