Roles & Responsibilities

• Deliver skilled physical therapy assessments and individualized treatment plans in patients' homes
• Provide hands-on interventions and progressive home exercise programs that help patients regain independence
• Document care accurately and promptly; Epic experience is preferred though not required
• Collaborate with nurses, physicians, and care coordinators to ensure a seamless continuum of care
• Educate patients and caregivers on safety, mobility strategies, fall prevention, and equipment needs
• Complete required competency testing and skills checklists to maintain high standards of care
• Maintain license and certifications, and comply with organizational health screening requirements

Qualifications

• Valid Physical Therapist license (state licensure required)
• Current BLS certification (required)
• Home health experience preferred but new-to-home-health clinicians encouraged to apply
• Epic experience preferred but not mandatory
• Ability to complete Trinity specialty competency testing and skills checklists
• Meet standard health and background screening requirements (vaccinations, drug screen, TB testing, etc.)

Knowledge & Skills

• Strong assessment and treatment planning skills for adult and geriatric populations
• Proficiency in functional mobility, gait training, and ADL-focused interventions
• Comfortable with home safety evaluations and recommending durable medical equipment
• Clear, compassionate communication with patients, families, and interdisciplinary teams
• Competent documentation practices and attention to regulatory compliance
• Ability to work independently in community settings and manage caseload efficiently
