Roles & Responsibilities

• Provide compassionate bedside nursing care to residents at Laconia Rehab Ctr, focusing on safety and dignity
• Administer medications and treatments accurately and document according to facility protocols
• Monitor and assess patient conditions, reporting changes promptly to the care team
• Collaborate with RNs, therapists, and support staff to implement individualized care plans
• Assist residents with activities of daily living and promote independence whenever possible
• Maintain clear, timely charting in the facility’s documentation system to ensure continuity of care
• Support infection control practices and maintain a safe, clean environment for residents

Qualifications

• Active Licensed Practical Nurse (LPN) license — state licensure appropriate for assignment
• Current Basic Life Support (BLS) certification or willingness to obtain upon hire
• Comfortable working overnight shifts and providing hands-on resident care
• Strong communication skills and ability to work collaboratively in a multidisciplinary team
• Previous experience in long-term care, rehabilitation, or skilled nursing settings is a plus

Education

• LPN diploma, certificate, or equivalent vocational nursing program completion

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment, vital signs, and clinical observation skills
• Familiarity with long-term care and rehab nursing routines
• Proficient in clear, concise charting and electronic medical records
• Strong interpersonal skills for working with residents, families, and care teams
• Infection prevention, wound care basics, and safe patient handling awareness
