Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and treatments safely and document accurately
• Monitor and record vital signs, patient status, and response to care
• Assist with activities of daily living and promote patient comfort and dignity
• Collaborate with RNs, physicians, and the interdisciplinary team to support care plans
• Maintain clear, timely charting in the electronic health record
• Recognize and respond to changes in patient condition, escalating concerns as appropriate
• Support infection control practices and maintain a safe care environment

Qualifications

• Current Licensed Practical Nurse (LPN) license (able to work in New Mexico)
• Basic Life Support (BLS/CPR) certification required or willing to obtain
• Comfort working overnight shifts and adapting to per diem scheduling
• Strong communication skills and the ability to work collaboratively
• Proof of required immunizations and ability to pass background checks
• Previous LPN experience in a long-term care or rehab setting preferred

Education

• Graduate of an accredited Practical Nursing program (diploma or equivalent)

Knowledge & Skills

• Solid knowledge of medication administration and documentation
• Patient assessment and fundamental nursing care skills
• Familiarity with infection control and basic wound care
• Comfortable using an electronic health record for charting
• Time management and prioritization during busy night shifts
• Teamwork and clear communication with patients, families, and staff
