Roles & Responsibilities

• Provide compassionate, direct nursing care to residents consistent with LPN scope
• Administer medications and treatments accurately and document per facility policy
• Monitor residents' condition and report changes to the care team promptly
• Collaborate with RNs, physicians, and therapists to deliver coordinated care
• Educate residents and families about care plans and promote independence
• Maintain accurate and timely clinical documentation in the medical record
• Follow infection control, safety, and facility policies to protect patients and staff

Qualifications

• Active LPN license (Licensed Practical Nurse) — New Mexico or eligible to obtain
• Current BLS/CPR certification (or willingness to obtain promptly)
• Comfortable administering medications and performing routine nursing procedures
• Strong communication skills and ability to work well on a multidisciplinary team
• Previous long-term care or rehab experience preferred but new grads welcome with appropriate support

Education

• Graduate of an accredited practical nursing program
• Current LPN licensure required

Knowledge & Skills

• Medication administration and basic wound care
• Patient assessment and vital signs monitoring
• Charting and documentation best practices
• Infection control and safety procedures
• Clear communication with patients, families, and clinical staff
• Time management and prioritization during 12+ hour shifts
